**Im in California Obviously, if only one parent is alive, then only that parent must sign. (Signatures of dead parents are not required.)
If both parents have joint custody, then both must sign. The form specifically says this. If one parent has sole custody, then the signature of that parent is required, but the signature of the other parent is not required. |
